You can also print and complete one of the questionnaires below. Please hand it to reception team.
- GMS1 Form (Mandatory )
- New Patient Questionnaire – Adult ( Mandatory)
- New Patient Questionnaire – Child ( Mandatory)
2. Register with a GP in Person
Prefer face-to-face registration? Visit our GP surgery in London and bring:
✔ Complete the registration form (available at reception or download the GP registration form).
✔ While not required, bringing proof of address and a photo ID can speed up the process.
After you have downloaded and complete your forms, please contact the practice to confirm the email address for these to be sent to. We will then ask you to attend the practice with two forms of identification to complete your registration. While not required, bringing proof of address and a photo ID can speed up the process.
Acceptable proof of address:
- Utility Bill (Gas, Electric, Water etc)
- Bank Statement
- Council Tax
- Tenancy Agreement
Acceptable photo identification includes:
- Passport
- Identity Card
- Driving License
- Hostel Registration/mail forwarding letter.
