How to register
We are happy to register new NHS patients within our practice boundary area.
You can also print and complete one of the questionnaires below. Please hand it to reception team.
- GMS1 Form (Mandatory )
- New Patient Questionnaire – Adult (Mandatory)
- New Patient Questionnaire – Child (Mandatory)
After you have downloaded and complete your forms, please contact the practice to confirm the email address for these to be sent to. We will then ask you to attend the practice with two forms of identification to complete your registration.
Acceptable proof of address:
- Utility Bill (Gas, Electric, Water etc)
- Bank Statement
- Council Tax
- Tenancy Agreement
Acceptable photo identification includes:
- Passport
- Identity Card
- Driving License
- Hostel Registration/mail forwarding letter.
Until we have seen your identification you will not be registered at the practice.
